You must log in by entering your license number and the last 4 digits of your social security number.

Review all information on each page carefully to ensure data accuracy.

If a field is blank, please complete if it applies to you.

All renewals will be reviewed prior to issuance of a renewal certificate.

Certificates will be mailed to your correspondence address. So please complete the correspondence address field to send your license renewal and any other mail we may send you where you want it to be sent.

If you answer yes to any of the questions on pages 3 or 4, you are required to follow up with written explanation and/or documentation by mail, email or fax. Your license will be delayed until such explanation and/or documentation is received.

If you are in default with Worker’s Compensation or Unemployment Compensation, your license renewal will be delayed.

ACTIVE DUTY MILITARY INFORMATION
If you are on Active Duty Military Deployment or an accompanying spouse of a military member on Active Duty Military Deployment you may qualify for a waiver of your renewal fee this year. Please complete the appropriate waiver form and attach to your renewal, without a fee, and return to this office for processing. DO NOT RENEW ONLINE IF APPLYING FOR AN ACTIVE DUTY MILITARY WAIVER! YOU MUST SEND THIS DOCUMENTATION TO THE OFFICE. Should you have questions, please do not hesitate to contact the office.

DENTAL HYGIENISTS EMPLOYED BY A PULIC HEALTH AGENCY
Dental Hygienists Employed by a Public Health Agency Can Now Pay a Reduced Fee for License Renewal. These Agencies Include, Federal, State or County Health Departments or Health Rite. Federal or State Prisons Do Not Qualify.
